Sub Sponsor Shift Request Form - Quantum Construction & Rail Services

Once you have submitted your request to sub sponsor via Sentinel, and returned the Quantum Sub Sponsorship agreement (see this page), you need to complete a shift request form (below) for every shift you require the worker for. We ask for at least 48 hours notice of a request, but on occasion we may be able to agree at shorter notice. You cannot deploy the worker until you have received approval.

Once the shift has been worked we require notification of the actual hours they worked using the Sub Sponsors Actual Hours Report form, and safety incidences or misconduct, within 10 working days of the last shift that week. This is so that we can continue to monitor fatigue accurately. If the actual hours worked are not supplied, we may have to withdraw sub sponsorship.

Quantum has set the following workingtime limits:

·Maximum shift length of 12 hours, with travel to be no more than 1 hour each way on top of shift.

·Maximum of 72 hours worked in any 7 day period.

·Maximum of 13 days continuous in any 14 day period.

·Maximum travel time, including shift not to exceed 14 hours
